Initial Nutrition Consultation

Are you sick, tired, and sick of being tired? Are you wanting to improve your health and address the root cause of your symptoms? When western medicine has few answers and doesn’t treat patients as a whole person, we look at the body differently and are passionate about your health and supporting you on your health journey!

Systems Survey Form, History Questionnaire and Lab work must be submitted 48 hours in advance in order to be discussed during the initial consultation. 

The Initial Nutritional Consultation is the start of our work together, to give you a full analysis of areas that functional medicine can best support you and give you a framework for the path forward towards better health! An Initial Consultation must be scheduled prior to follow up muscle testing or subsequent visits. 

Cancellation fees: We are reserving resources and time for you. Please give us at least 24 hours notice if you can’t make it. Appointments canceled with more than 24 hours notice incur no charge. There will be a $35 fee for appointments canceled with less than 24 hours notice. A full office visit fee will be charged for failure to provide any notice of cancellation or if payment is not made, future service bookings may be forfeited.
